New Employee Request Form

This form is used for requesting that the IT Division setup a new employees with access to various systems, email, and/or a ShoreTel phone. This form is not for requesting a computer or cell phone for a new employee. There are other specific forms for those purposes.

Do not fill out this form until you are 100% certain of proper employee name spelling. 

Under normal circumstances, IT can complete a new employee setup in about 5 working days. Incorrect submissions will take extra time to fix and likely delay the process of getting the new user setup. Thank you for your understanding in this matter.

 

  • Please Note: The "New Employee's Full Name" must be exactly as HR has it spelled.
  • Please Note: The New Employee's Display Name is the way the employee wants to be shown on the ShoreTel phone system and in the employee directory, and is the way his or her email is shown.

    For instance, if you enter "John Doe" above, "John Doe" would display on the ShoreTel phone sytem and in the employee directory, plus his email would be john.doe@cityofsanrafael.org.
