Human Resources is ready to begin accepting gym reimbursement requests via our new online form/process: Gym Reimbursement Request Form.
The reimbursement period is for any gym/fitness fees paid from July 2023 to June 2024.
A few friendly reminders about the gym reimbursement benefit:
- This benefit is available to regular full-time employees (Temp, Seasonal, and Fixed Term employees are not eligible; Police employees have a separate process for reimbursement)
- The maximum annual benefit amount is $198 (prorated at $16.50/month if you were not a regular full-time employee all 12 months of the fiscal year)
- Your reimbursement request will be expedited if you’re able to provide your employee # (you can find this at the top if your timesheet in HR Web)
- The receipts/backup documents you provide must include your name, the date of your purchase/payment, the amount you paid, and the company payment was to
Reimbursement requests *must* be submitted via the form linked above no later than Monday, July 1st. Late submissions cannot be processed/accepted. Reimbursements should be included on your July 15th paycheck.
